Insurance participation is handled through Tebra. Because network status can vary by state, employer plan and specific product, the most accurate way to confirm coverage is to enter your insurance information directly through Tebra.
You will be able to review available insurance based scheduling options and, when available, an estimate of your patient responsibility before your visit.

Virtual psychiatric services are available to patients who are physically located in an eligible state at the time of the appointment, subject to clinical appropriateness and applicable licensure requirements.
Transparent pricing for the time and depth your care requires.
Private pay fees reflect appointment length and service type. The fee for your scheduled visit applies to the full reserved appointment time.
Rates are subject to change with advance notice when applicable. The practice may offer a limited number of sliding scale appointments based on clinical availability and financial need. Please contact EMPATH Medicine for questions about the appropriate appointment type.
Clinical work outside your appointment may require additional time.
Some requests require physician review, record review, care coordination, form completion or other work performed outside of scheduled appointment time. These services may carry an additional fee based on time, urgency and complexity.

EMPATH Medicine participates with select commercial insurance plans through Tebra. Network participation can vary by state and by the exact plan you have, so the most accurate way to confirm coverage is to enter your insurance information through the Tebra booking link.
Private pay is available for patients who prefer to self pay, are not eligible for an in network option, or are seeking services that may not be covered by their plan. If you have insurance, you may wish to review your benefits before deciding.
Yes. A superbill can be provided upon request for private pay services when appropriate. Whether your plan reimburses any portion of the visit is determined by your insurer and is not guaranteed.
Many forms, accommodation letters, disability requests and insurance related tasks require chart review, clinical judgment, documentation and physician time outside of your scheduled appointment. Fees reflect the time and complexity involved.
Cancellations with less than 48 business hours notice are charged 50% of the scheduled fee. Cancellations with less than 24 hours notice and missed appointments are charged the full scheduled fee, subject to clinician discretion in significant unforeseen circumstances.
Insurance based care is billed according to the applicable insurer contract and your individual plan benefits. The private pay fees shown on this page apply to self pay services and should not be used to estimate an insurance copay, deductible or contracted rate.
